1. Use of Service
FORMA is an AI-powered SQL generator that converts natural-language questions into SQL queries. You agree to use the service only for lawful purposes and within applicable data regulations.
2. Accounts
You are responsible for maintaining the security of your credentials. You may not share, resell, or misuse your access.
Subscription Terms: FORMA SQL is a monthly subscription service. Founding Members (first 100 customers) pay $59/month with a lifetime price lock—this rate never increases. Regular members pay $99/month. You may cancel your subscription at any time by emailing support@formastudios.ai. No refunds will be issued for partial months or cancellations.
3. Intellectual Property
All software, design, and content in FORMA are owned by FORMA Studios LLC. You may not copy, reproduce, or redistribute without written consent.
4. Payment and Access
All sales for the Founding Member Lifetime plan are final. Future subscription tiers may be billed monthly or annually via Stripe. Pricing is subject to change.
5. Disclaimer
FORMA is provided "as is" without warranty of any kind. We are not responsible for losses or errors from generated SQL output or user-supplied data.
6. Limitation of Liability
To the fullest extent permitted by law, FORMA Studios LLC is not liable for indirect, incidental, or consequential damages.
7. Termination
We may suspend or terminate accounts violating these terms.
8. Governing Law
These terms are governed by the laws of Arizona, USA.
